New Patients
New Patient Registration Forms MUST be Completed Before Your Appointment
Before downloading or filling out the new patient forms, we kindly request that you first read, understand, and acknowledge the Total Health Dentistry Hawaii Patient Bargain below to ensure clarity and alignment with our cooperative approach to dental care.
THE BARGAIN
Our Mutual Commitment to Your Dental Health
At Total Health Dentistry Hawaii, we believe that achieving optimal dental health is a partnership. When you choose us to help you with your care, it’s important that we both understand and agree on what we can expect from each other. We see this as a “bargain” between us—one that sets the foundation for trust, respect, and successful outcomes.
What You Can Expect From Us
1. Personalized Care
We take the time to understand your unique needs, preferences, and dental goals. You’re more than just a patient to us, and we will work with you to develop a customized Master Plan for your dental health that suits your individual situation.
2. Respect for Your Time
We value your time and strive to run on schedule as much as humanly possible. If we ever need to reschedule your appointment (for instance, if Dr. Hartwell is called away for teaching or continuing education), we will give you ample notice—typically several weeks in advance.
3. Transparent Financial Discussions
Before performing significant dental procedures, we’ll discuss all associated costs upfront. We believe in transparency and will never spend your money without your explicit consent.
4. Focused, Undivided Attention
When you’re in our care, we’re fully present and focused on you. We minimize non-essential interruptions such as phone calls to ensure that you have our complete attention throughout your visit.
What We Expect From You
1. Commitment to Your Dental Health Plan
Once we agree on a Master Plan to improve your dental health, we expect that you’ll follow through at a pace that works best for you. Your commitment is essential to achieving the best results.
2. Respect for Appointment Scheduling
We ask that when you schedule an appointment with us, you do so at a time that you are fully confident will not need to be changed. Last-minute cancellations or frequent appointment changes disrupt our ability to provide high-quality care. If scheduling conflicts become a habit, we may ask you to consider finding another practice to meet your needs.
3. Honor Financial Agreements
We trust that you will respect and fulfill the financial arrangements made with our office in a timely manner.
4. Complete Focus During Treatment
We ask that while you’re in our care, you give us your undivided attention. Please turn off or ignore your phone while in the treatment areas to ensure a calm, focused environment.
A Partnership for Success
This mutual agreement is designed to help us provide you with the best dental care possible while ensuring that we respect one another’s time and commitment. By working together under this “bargain,” we can help you achieve lasting dental health.
If you have any questions about this agreement or would like to begin your journey to better dental health, contact us today to schedule your next appointment.
New Patient Forms
To provide you with the best care, we ask that you complete the following onboarding forms. Your detailed dental history is especially important if you’re facing specific issues. Please share as much information as possible about your past dental experiences, including treatments that have been effective and those that haven’t.
